Research and Survey:
Conduct extensive research and surveys among individuals aged 50 and above to identify interests, unrecognized health activities, and service gaps. Address access challenges due to socioeconomic factors.
Database/Resource Center:
Establish a comprehensive database/resource center for Vermont adults aged 50 and above, housing information, organizations, and services supporting older Vermonters.
Online Presence:
Develop and maintain an interactive website as a hub for information, organizations, activities, and services tailored for Vermonters aged 50 and above.
Resource Guide:
Organize and publish a detailed resource guide containing essential information to enhance the lives of Vermonters aged 50 and above.
Partnerships and Activities:
Forge strategic partnerships with conservation and recreational organizations. Collaborate on initiatives aligning with the interests of individuals aged 50 and above, promoting healthful aging by accessing public and private resources.
Create Education Programs:
Develop and implement curriculum to educate and empower Vermonters 50+ on resources available for health, wellness, and financial security.

Growing, Healthy, Active People (GHAP) Program
Imagine you have just learned that your A1c is 5.8 and you have been recommended to lose weight. Your doctor refers you to a dietician who can provide some nutritional guidance. You set the appointment for 3 months from now and you’ve learned that your insurance will pay for 3 appointments. Until then, you look diabetes up on the internet and get all kinds of information so that you can start working on your diet. Weeks later, you’ve lost a few pounds but not what you expected. Then there is a family event during the weekend, you gain the weight back. Finally, the day of your appointment with the dietician arrives! You get a few tip sheets, some information about carbohydrates and you have a sense of what you can do. You set another appointment for 30 days out. 30 days is a long time for someone who is unsure of their ability to manage the task of weight loss to offset such a big health issue.
The medical system is not designed to provide daily accountability and oversight. That’s where GHAP comes in! Mind, Heart, and Muscle is seeking a way to help people as described above to understand their body’s need for nutrition, movement, and support better so they can become healthier and happier versions of their current selves.
Qualified personnel will work with participants to design personal nutrition, movement and support plans so that changes can be as simple or complex as one can manage. There will be weekly meetings to discuss goals and issues affecting progress, to take measures, blood pressure readings and to monitor blood sugar readings depending on what is needed. And, we don’t work in isolation as your physician is a necessary part of this process.
